This policy explains what information this website collects, why, and what we do with it. It covers the website. Medical information collected during your exam at the clinic is governed by the rules that apply to medical records.
What we collect
The appointment request form collects your full name, date of birth, email address, phone number, and the type of immigration application you are making.
The form asks for no medical information. Please do not send health information through it.
Why we collect it
Only to contact you and arrange your appointment. We do not use this information for advertising and we do not sell it.
Who receives it
Your request is emailed to the clinic using Brevo's transactional email service, acting as a technical provider. The site is hosted by Vercel. These providers process the data on our behalf and may host it outside Québec.
Cookies and analytics
This site uses Google Analytics to measure traffic. Google Analytics sets cookies on your device and collects usage data, including a truncated IP address.
You can block cookies in your browser settings, or install Google's Analytics opt-out add-on. The site remains fully usable without them.
Retention
We keep appointment requests for as long as we need them to arrange your appointment and keep an administrative record, then we delete them.
Your rights
You have the right to access the personal information we hold about you, to have it corrected, and to withdraw your consent. Write to cliniqueluso@yahoo.ca or call (514) 849-2391.
If our response does not satisfy you, you may complain to the Commission d'accès à l'information du Québec.
